General treatment for Chronic Leg Ulcers

Effective treatment starts with accurate assessment

For effective healing of leg ulcers a detailed wound assessment must first be completed, usually using a combination of methods including:15,16

  • Reviewing a medical history
  • Ankle-Brachial (pressure) Index (AB(P)I)
    • Values less than 0.8 identify patients with peripheral arterial disease (according to the Scottish Intercollegiate Guidelines Network - SIGN)15
  • Varicose Veins-Doppler Exam
    • Color Duplex Ultrasound Exam
    • Hand Held Continuous Wave Doppler
    • Venous photoplethysmography (PPG)
    • Venography
  • Ulcer characteristics
    • Size, edge, base, position of the ulcer and its surrounding skin.

Following the assessment, a treatment plan will be formulated with the following goals in mind:16

  • Reduction of oedema and/or swelling
  • Pain reduction
  • Improvement of lipodermatosclerotis
  • Ulcer healing
  • Preventing recurrence (by reducing venous hypertension, increasing venous return, increasing fibrinolytic activity in the tissues and improving oxygenation)
